Scarborough Health Network (SHN) and our Scarborough Ontario Health Team (SOHT) and local partners have come together to rollout the COVID-19 vaccine across our community. We are doing this through vaccine clinic sites and mobile vaccine teams. We are pleased to be keeping our community safe and healthy.

Pediatric Vaccine (1st & 2nd doses)
- Children aged 5 to 11 years old
- Parent/Guardian must accompany child
- Child must not have received any other vaccine in the last 14 days
- The Ontario Ministry of Health recommends 8 weeks between the first and second dose
- For 2nd doses: May receive 2nd dose if it has been at least 21 days since 1st dose, and with parental consent
4th Dose Boosters
- Individuals aged 60 years or older
- First Nation, Inuit and Métis adults and their non-Indigenous household members who are 18+ years of age
- Must have received 3rd dose at least 5 months ago
- May receive 4th dose booster if it has been at least 3 months since 3rd dose, and with patient’s consent
- Individuals who are moderately or severely immunocompromised
- Must have received 3rd dose at least 84 days ago
3rd dose Boosters
- Individuals aged 12 years or older
- Pfizer boosters (3rd doses) available for those 12+ years old
- Moderna boosters (3rd doses) available for those 30+ years old
- Ages 12-17: Must have received 2nd dose at least 6 months ago, or 3 months (with informed consent)
- Ages 18+: Must have received 2nd dose at least 3 months ago
- Individuals who received a complete series of a viral vector vaccine (two doses of the AstraZeneca vaccine or one dose of the Janssen vaccine) must have received 2nd dose at least 6 months ago
1st and 2nd doses
- Individuals aged 12 years or older
- Pfizer available for those 12+ years old
- Moderna available for those 18+ years old
- For 2ND dose: Must have received 1st dose of Pfizer 21+ days ago or Moderna 28+ days ago
- Must provide a form of photo ID with name and birthdate (such as a government-issued ID, if you have one)
- No health card, no problem
- For your 2nd and 3rd doses, please bring your vaccination certificate or the vaccination receipt for your 1st dose, if you are able (download a copy from the Ministry’s COVID-19 Vaccination website). Showing it on your phone is fine!
- If your first vaccination dose or doses was with a non-Health Canada approved vaccine, please bring proof of your vaccination status
